Letter to Parents

Preparticipation Physical Evaluation – EL2
Consent & Release from Liability Certificate – EL3

These two forms required by the FHSAA before a student-athlete can participate in any type of conditioning, workout, practices or contests are ONLY AVAILABLE AT EACH HIGH SCHOOL IN THE OFFICE OF THE ATHLETIC DIRECTOR.  These two forms must be turned in directly to the Athletic Director.

These forms contain additional statements which are specific to the School District of Polk County which require the signatures of the parent/guardian and student-athlete.

For that reason, please do not download the EL2 and EL3 forms from the FHSAA website.

Parents must have the physical paperwork completed prior to arriving with a parent’s signature. We will not be able to complete the physical if the paperwork is not signed.

We are very pleased to announce that Polk County School Board has elected to purchase an athletic accident insurance plan for the 2009-2010 school years.  NEW! Important Information – Student Accident Insurance

  • The Student Accident Policy will provide basic insurance protection for all full time students in Pre- Kindergarten through 12th grade for school related accidents during the regular school term.
  • There will be no enrollment forms to fill out – schools will not have to collect insurance premiums.
  • The policy contains limits and coverage restrictions -it will not pay 100% for all medical expenses incurred -it does not guarantee coverage for all situations.
  • Please refer all questions regarding claims and coverage to the company that will be processing the program for us, School Insurance of Florida.
